Every Hudson student-athlete must have the required health and eligibility paperwork on file before participating. Below are the materials required by the New Jersey Board of Education.
Healthy bodies and minds come first
The Athletic Department focuses on building healthy bodies and minds. Before a student takes the field, court, or track, New Jersey requires certain health forms and educational materials to be completed and reviewed. Athletic Participation Acknowledgement Form
Every student-athlete, with a parent or guardian, completes this acknowledgement before the season begins.

Participation Physical Evaluation
The Medical Eligibility Form must be completed by a licensed physician before a student may participate in athletics.

New Jersey requires student-athletes and families to review this fact sheet on recognizing and responding to concussions.
